At Groups, we recognize that every data point tells a story — a life changed, a future reclaimed, a family reunited, and a community restored. We are deeply proud of all that our members, team, and community have accomplished.

Our third Annual Outcomes Report highlights how our model creates lasting change through six Core Outcomes that define success in addiction treatment:

Access & Growth
Expanding care by reducing barriers for vulnerable populations. In 2025, 21,315 individuals scheduled intake appointments with a median wait time of just 1 business day.
Engagement
Keeping members actively involved in treatment through community and personalized support. Our community-powered care model drives 93% monthly engagement.
Retention
Higher treatment retention rates highlight Groups' ability to support sustained recovery: Groups achieved 6-month retention rates that were 3 times higher than the national benchmark, a key clinical milestone.
Remission
Helping members abstain from or reduce opioid and other substance use. Remission rates underscore the value of sustained engagement: Groups Members retained for 180+ days achieved near 100% remission as defined by 90 days of treatment without the use of illicit opioids compared to 33% nationally.
Mortality Reduction
Providing life-saving medications, tools for overdose prevention, and assistance with safety planning. Significantly reduced mortality rates: In 2025, Groups’ mortality rate was more than 3.5 times lower than the national benchmark for individuals treated for opioid use disorder with buprenorphine or methadone, and 8 times lower than for individuals with OUD who were not in treatment.
Cost of Care
Lowering overall health care costs by providing better care. Meaningful cost savings: Claims-based studies demonstrate that Groups achieved 30-40% lower total cost of care compared to other outpatient and residential treatment options. This is due to higher retention rates and reduced reliance on costly emergency and inpatient services.
A Message From Our CEO

At Groups, we change lives.

The addiction landscape is shifting. For the first time in a decade, we're seeing meaningful declines in overdose deaths. Overdose mortality decreased by 21% nationally between August 2024 and August 2025. Yet the overdose crisis is far from over. Against the backdrop of rising acuity in our population, we continue to operate in a dynamic and complex healthcare environment, especially for Medicaid enrollees.

Our mission has never been more important. In 2025, we continued to invest in what works, while developing new ways to strengthen our industry-leading outcomes and deliver more value to our members, our communities, and our partners.

In 2026, the landscape will continue to evolve, and we will evolve with it. What will never change is our mission, our optimism, and our belief that every person deserves the chance to recover with dignity. Thank you for your commitment to this work and the people who depend upon us. The future of Groups is bright, and I’m honored to lead us forward.

-Cooper Zelnick, CEO of Groups
